The HaveCare app addresses challenges facing patients living with chronic diseases in low and middle income countries and offers numerous opportunities including improved; access to the services wherever they need them, verified health providers, linkage and referral, access to trends of patient’s vitals and treatment provided and predictability of services/price structure.
The App also offers various payment options, ensures guaranteed payment to the health worker and continuous evaluation of health-providers.
The Users of the application are categorized into two;
1) Health Care providers (caregivers) these are majority nurses, midwives, counsellors and physiotherapists.
2) Health care consumers – these are either patients or their guardians.
The services offered through the App are; home care, palliative care, counselling, nutrition support, physiotherapy, guidance and counselling, drug administration, adherence support, wound management and post-surgical operation support, diagnostic services - blood glucose and other tests. Routine visits for monitoring of vitals including Blood pressure will be provided to ensure close monitoring of medical conditions.
The role of caregivers is to;
Provide outreach to individuals in the community setting.
Measure and monitor vitals (blood pressure, Hearth Rate, Weight etc).
Educate patients and their families on adherence to their medication treatments.
Help patients navigate health care systems.
Provide social support by listening to the concerns of patients and their family members and helping them solve problems.
Create community-clinical linkages to create a team-based approach to care.
Supporting patient self-management plans and long-term self-management support.
